Can I get your opinion on using Meditech to enter QC. We are currently using a paper log and we are going live with Meditech in Aug. Meditech has yet to show me what it will look like to use the system to perform QC. Will it be worth the trouble to use Meditech for QC?

Our hospital is very small and we use Meditech. Our providers order cord blood work ups on all babies regardless of the mothers Rh typing. They say it is because they do not want to miss one. I can appreciate that. I was wondering how other Meditech users manage the ordering of cord blood, so that a baby of an Rh negative mother does not get missed, and allow babies of Rh positive mothers to not routinely be tested? I asked Meditech and they could not help me. Thanks in advance

We have Sunquest 6.4 for our LIS and i-Extend/Cerner on the HIS side. I want to create electronic documents on the HIS side for 1) uni tag/transfusion report form for documenting start/stop/vitals/etc. 2) transfusion reaction orders/documentation/clerical checks/results 3) "electronic" blood request slip. I am looking for "best practice" so as not to reinvent a good process. Mary Ann Womack, Florida Hospital Maryann.Womack@flhosp.org

Our blood bank is exploring a move to Softbank. However, the build quote only includes the upload and verification of archival data. We have to supply them with the archival data. One route to obtain this is "through internal IT resources." Wondering if anyone has any experiencing extracting data from Safetrace/HBB for use in Softbank.

I'm curious how expiration time for units is displayed to users in different time zones. What is the philosophy behind this? If I have a thawed FFP that expires at 7/25/24 15:49 and my user is CST, would another user viewing this units information from EST see 16:49 or 15:49? Are 23:59 expirations just assumed end of day and never change but units with explicit expirations treated differently?

So - just curious to hear from current Sunquest users......(it's changed names - but not sure what the new name is..... Clini.........?) We had Sunquest years ago and then moved to SCC (SoftBank). SCC offers things like Transfusion Reaction documentation capabilities, Reagent QC documentation, supply inventories which - back in the day - Sunquest did not. Our facility is most likely going back to Sunquest.......not really by "our choice" but, through our hospital network choice. The question is, does Sunquest now offer the above listed capabilities in any form or fashion?

Has anyone found software that can help them digitize all the paperwork (antigrams, instrument printouts, etc) for patient antibodies? I have a HUGE file cabinet full of these things, and we'd like to make it electronic, but of course it has to be searchable by name and/or MRN and we would need access to it during downtimes. I'm thinking a third party software out there must have this capability by now....it is 2024 after all! We have Softbank and Epic, and I'm aware that Soft has this capability, but it will cost us upwards of $20K to complete the project, and no one has that just laying around these days.

Hello, at our facility, we send any positive DAT(we use polyspecific) on cord blood to reference lab for work up/elution for HDN. my question is, what should be done with mother/ should a new type and screen be performed soon after cord blood collected? our procedure says if moms antibody screen is positive, start HDN workup. so should it be a recent antibody screen? or prev prenatal antibody screen result? confusion regarding mother, if antibody screen negative don't send? could an antibody eluted off be missed if moms antibody screen was negative? I am not original author of procedure, originally placed into service in 2006, biannually reviewed. and I am a recent h…

Hello Group, I am new to the transfusion area and just came across this document. I would like to know what is the process to determine if the recently released FDA guidance (regarding ISTBT v4.0.0) is applicable to blood bank lab. Thank you and appreciate any help.

Hello For Pre-Op patients not transfused or pregnant we will add a comment to GS ...Reserved until (60 days from collection) . For all other patients we are will not add a Reserved until comment ..we will choose to let the specimen be invalid for testing 96 hours post collection. (Technologist has to calculate 96 hours from collection.) In the event a nurse or clerk enters the wrong history , we won't have an issue as we obtain a new sample. Question..Are there Meditech 6.16 users who do things differently? Allow some samples to be counted 96 hours from issue if history is taken and trusted. Thanks Richard

I find myself involved in a variety of projects integrating the lab with other systems. I’ve become familiar with HL7 v2, but have heard a fair amount about FHIR (Fast Health Interoperability Resources). I tried to read the specs, and i can see some of the topics I would want to understand (RESTful, “Smart-on-FHIR”) and questions I might ask (all or none? use cases?) What I can’t find is something in the Informatics world going to the middle level understanding. Is there an updated textbook or general review that someone might recommend for readability? Thank you Keith Krabill, MD Buffalo, NY

Does anyone know of a scanner that is good for scanning request forms (including the flimsy multi-copy ones)? And also software that will identify if two sheets are being taken through together and also direct the form to the patient record. I've used Dart in the past which was OK but I'm wondering if there is anything better out there?

Good morning friends! I hope everyone is having a great week! We are currently in the process of converting to EPIC/Soft. Does anyone use Rover? Does it cover the AABB standard requirement 5.14.1 which states: 3. Retesting the same sample if patient identification was verified at the time of sample collection using an electronic identification system. Thank you! Have a great day! Sara

Does anyone have experience moving from Sunquest to Wellsky? We have been on SQ since approximately 1990 and recently the laboratory had Epic Beaker implemented. Did not go smoothly and I am concerned about the Epic / Wellsky partnership. Some of the SQ functionality was removed going to Epic Beaker and I have concerns that Wellsky may also affect the department operations. I would greatly appreciate hearing from anyone on how the implementation went as well as the validation. Thank you all in advance! Oh, by the way, we are a 600 bed hospital, with a high risk OB, NICU, PICU, high risk heart program and a large ortho program. MTP will also be implemented sometime in J…

I've received a lot of complaints on how transfusions are tracked for the doctors and nursing teams. Those of you on Epic, how are you tracking transfusions as far as start/end and vitals? One request was to add it to the MAR as a medication. This looks like the transfusion tracking utilized in Sunrise Clinical Manager. It's a scrolling timeline with meds listed vertically. I know Epic is super customizable so there has to be a better way than what we are using.

OK Question for you... I have always reactivated accounts to LIMS and QMS if the leaver has returned whilst still under technical competency (up to 2 years). ISO 27001:2016 - A.11.5.2 User identification and authentication Control - All users shall have a unique identifier (user ID) for their personal use only, and a suitable authentication technique shall be chosen to substantiate the claimed identity of a user I have interpreted this as "as long as we can confirm the identity of the staff member (which we can), then there are no issues with reinstating their login. Having one person with two logins recorded against them means there is no longer a un…

When Meditech 5.67 is experiencing a downtime event, how do you make patient records accessible for techs to perform record checks? The NJ state requirements are - "Before blood is issued for transfusion, test results for each recipient sample shall be compared with the following: 1. Past records of previous ABO and Rh typing results for the past 12 months; and 2. Past records of all patients known to have significant unexpected antibodies; severe adverse reactions to transfusion, and/or difficulty in blood typing." If computers are used, an alternate method shall be available and used which allows access to the information required in above in the case of computer failur…

Hi! I am here as a desperate last minute. We are days away from our go live for EPIC Beaker for anatomical pathology and are having several issues with no help of IT or the build team. Basic function is manageable, although, I have questions regarding batching specimens and choosing which processor the specimen was placed on. Also, formalin fixation times. I see where there is an option for in and out fixative times but unsure if there is a way to see the formalin fixation time somewhere. If anyone has a build specific and can lend a helping hand it would be much appreciated.

My hospital recently went live with EPIC’s BPAM module, because of which we had to increase the amount of data being sent across the interface (more OBX segments cross into EPIC for the patient/product matching aspect). The Transfusion Service uses Softbank and we've discovered one the main Soft interfaces into EPIC can only process so many lines of data at one time. How did we discover this? We broke the interface! One massively transfused patient sent across so much data that it crashed. Soft is currently working on a fix for this, but the main workaround they gave is to limit the number of products you can crossmatch/dispense on each order (rec…

Hello, I was wondering who uses Sunquest with EPIC (hospital EMR) with the BPAM (blood product administration module). Looking to get some insight for validation in blood bank, rough patches that were encountered, barriers that were overcome to go live. Thanks :-)
