Kathyang got a reaction from Annjojo in ABO verification process with the use of SOFTIDWe have always asked for another tube when a patient doesn't have history when using electronic ID. We know that the electronic ID is good but there are ways to trick it so they can san something besides the bracelet. I have used 3 different systems with electronic ID and nurses still try to get around using the proper scanning technique. We got support from our Medical Director and we just did it We also have a second person draw the tube, not the same nurse or phlebotomist.

We provide blood to a cancer center in a town about 15 miles away. There is another hospital in the town that is in a different system. We provide the blood to the cancer center. We ship it in coolers through our courier system and then the cooler comes back when the courier stops. We also supply them with platelets the same way. This way we keep all the blood under the same billing number.

Kathyang got a reaction from Ensis01 in Patient re-typeThis is a CAP and AABB requirement. We get a new tube on any inpatient, OR patient, or ED patient whether they are getting blood or not. That way if they do need blood, we already have the second type. The tube should be drawn by a second person at a different time.

Kathyang got a reaction from AMcCord in Emergency ReleaseWe have this with Epic and it does save time in an emergency or MTP. Also we don't have to worry about the paperwork, it is right in the EMR. We still ask for the call since we are the ones that are usually ordering the Emergency transfusion.

I worry since I have seen it too often someone using a relative's Health Insurance Card and having a complete different type. We don't need a specimen though form 3 days. If they have had a specimen during the stay, we will thaw plasma.

Kathyang got a reaction from David Saikin in Blood bank bands/ Typenex bandsIf you use Soft Transfusion package instead of Epic's transfusion then I would be comfortable not using a band. Soft ID for Transfusion is great.

Kathyang got a reaction from Ensis01 in LIS Product CategoriesWe have all of the units listed as you mention above in the Blood Bank system but the Dr's only see the 4 main categories. If they want anything special in the unit (irradiated, washed, etc.) they have to use a special comment. This way they don't get mixed up on which product to select.

What is the statistical likelihood of her future pregnancies coming to term w/o HDFN if there is NO fetomaternal hemorrhage during those future pregnancies?Just for knowledge, my mother-in-law had 6 Rh positive kids and she was Rh- Negative. There was no RhoGam back then. She came in later to have surgery and still had no Anti-D. She was not weak D positive either.

Kathyang got a reaction from David Saikin in Blood products for ORWe treat the OR patient like any other patient that gets blood. They can ask for an emergency release but we still require them to bring a piece of paper with patient's name, medical record number, date of birth, and Blood Bank ID if the patient has a BBID wrist band. They may bring a chart sticker down to get blood since it has all but the BBID number on it. The BBID number must be written on the piece of paper. They don't always like this process but we need to know which patient since they will probably have more than one patient. It is our way to cover for legality purposes.

I have a separate one for evenings and nights since they don't do all the same testing as days.

They will get their own specimen and would never use ours. I know we would not accept a specimen from another facility. I would do an emergency release first. I agree that specimen identification would be a big danger. I would think that you could say no just as a patient safety issue or do an emergency release if the blood is needed that quickly. That way the Blood Bank would be covered is anything happens.
