    Peter was always helpful, gracious and supportive of everyone in the field.  He made great contributions, not least of which was his definitive book Applied Blood Group Serology, which was recently reprinted.  A fabulous reference for the ages. Doubt it will ever be replaced or exceeded.  Hail and farewell.

    It is with huge regret that I can reveal that Marilyn Kay Grandstaff Moulds has just informed me that Peter Issitt died on Wednesday evening.  One of the truly great blood groupers.

    The trouble was that, in those days the anti-D immunoglobulin was known as "anti-D for Mum's Bums" in the UK, as the shot was given in the gluteal muscle.  But, there was an awful lot of fat in that muscle, so the anti-D had a habit of "staying there", rather than being adsorbed into the blood stream.  This meant that, even when the dose of anti-D immunoglobulin was calculated from the Kleihauer-Bekte test, the actual dose reaching the circulation was far lower than the calculated dose, and women used to produce allo-anti-D as a result.  Nowadays (at least in the UK) the shot is given in the lateral deltoid muscle, where there is a good deal less fat, and so the shot is adsorbed into the circulation much easier, and so there are fewer cases of maternal allo-anti-D.

    I realise that this is a very vague explanation, and that there are many other causes of anti-D immunoglobulin being less than effective (such as giving it to the father, or even to the ambulance staff (SHOULD be unbelievable, but is actually true), but it does show just how complicated such a simple thing as this can be.

    I know the feeling.  I once cross-matched for a patient who was a Jehovah's Witness who said that they had never been transfused.  The only trouble was that their serum/plasma contained one of the strongest anti-Fya's that I ever saw throughout my career!!!!!!!!

    I retired at the end of 2020.  I spent my last 25 years of my career working primarily in blood bank, including some time at an IRL. I always loved antibody ID.

    I worked in blood group serology/red cell immunohaematology for 43 years, until I retired in October 2016, but still love the subject - so we should get on well!!!!!!!!

    I once got a pre-surgery form where the patient initialed “never been transfused”. Our facility had given 5 RBC (over a two week period) three months previously.

    Ask the patient if they have ever been admitted with another name. We had a woman going for heart surgery and she told the phlebotomist she had antibodies. I asked her where they had been identified. She said this hospital. We could not find any records. When the antibodies were identified she had been admitted with a different name. No computer then. Could not search by MRN. (Note: it does not help when they have been given a new MRN, happens more frequent than I like judging by the number of patients I had to merge when I was in IT.) We had to go to medical records and search the microfiche. She had 3 clinically significant antibodies. Sorry, I don't remember the specificities.

    Ugh, sorry about this.  I made the bad decision to update the software just before my license ran out.  THEN they release a patch for this issue.
    I need to renew the licenses and install the patch, I'll take care of it soon.

    My own experience with an antibody card that would have been useful, from 2007.  Slides 5 and 25 are of particular interest from my own point of view!  It should be noted that I no longer work for the NBS/NHSBT, and am no longer entitled to put CSci FIBMS after my name, although I am entitled to put FBBTS after it.  Funny old world!
