tricore got a reaction from BldBnker in Antibody Screen before Issuing RhIg
This is from The Joint Commission 2011. I don't think it has changed, but, I don't have the latest edition of the Laboratory Accreditation Program for Blood Transfusion Service.
"The laboratory has written policies and procedures for the blood transfusion service that are consistent with AABB standards."

tricore got a reaction from MaryPDX in ISBT ZT410 printer
Is your printer only 203 dpi. Ours are 300 dpi. I am looking at the manual. It says accepted value for ZT410 Print Width 203 dpi = 0002 to 832. I don't see anything about max for label length.

tricore reacted to Malcolm Needs in Weak D+ specimens
I am sorry, but this is so wrong on so many levels. A Weak D is not just a mixture of D Negative red cells with a few D Positive red cells to give a weak mixed-field reaction. The various Weak D types have individual genetic mutations from the wild-type RHD gene - and it is these mutations that cause the weakness of the reaction. Using a genuine Weak D is the only solution, as the reaction in this case, as with all antibody/antigen reactions, follows the Law of Mass Action.

tricore reacted to Malcolm Needs in Antibody Titers
I agree, BUT, if the antibody identified in the previous sample has, for example, a specificity of anti-E, and you titrate using r"r red cells and get a titre of, for example, 64, and then the next sample gives a titre of 4, what do you do? At this stage, you have no idea whether the original titre of 64, or this titre of 4, is correct. It could be that there is a second specificity present (say an anti-Swa) and it just so happened that the original r"r red cells also expressed the Swa antigen. Now, I will readily admit that both anti-Swa and the antigen Swa are exceedingly rare, so these were probably not a great example, but, supposing it was an anti-E and an anti-Kpa?

tricore reacted to Malcolm Needs in Labeling Blood Components with Compatibility Info
I think those who do this would put the compatibility label on the back, so that no other labelling is covered up. The adhesive used should be tested to ensure that the label does not fall off between the temperatures at which the blood is stored or at the temperature at which the blood is transfused (and, in my experience, it is harder to get the labels off than it is to get a senior member of staff to buy a round of drinks!), but you do have to ensure that the adhesive is of a type that does not leech through the plastic into the unit itself (seriously - there are some that do)!

tricore reacted to Cliff in ABO/Rh confirmation labels--necessary??
I know this post is almost 4 years old, but it was quoted a couple of hours ago and I find the highlighted part interesting.
Not sure how big of a facility you are, or how much you transfuse. We're decent size, we transfuse about 24 - 28k RBCs a year. I've been at my current location for 26 years. That's approaching three quarters of a million red cells we've retyped in my time there. I recall only one unit we received from a facility that was mislabeled. That's pretty good evidence that retyping, while required, does not add a lot of value. The odds that something would go wrong are beyond astronomical. You'd have to have a mislabeled unit, you'd be on downtime, it would mistakenly get into the retyped area, then it would have to go to a patient where it could cause harm, then the odds that it actually would cause harm are still small.

tricore reacted to Likewine99 in ABO/Rh confirmation labels--necessary??
Same as David, quit using them about 20 yrs ago. They do not add any value to the retype process and they usually ended up falling off and sticking on the refrig, us, the floor, etc.
This of the money you will save not buying labels and with the time saved you might have time to get to lunch/dinner on your shift

tricore reacted to David Saikin in ABO/Rh confirmation labels--necessary??
We haven't used them in decades . . . even now when we no longer have a BBIS they are not necessary. Even before BBISs we never used them.
-
tricore reacted to Dr. Pepper in ABO/Rh confirmation labels--necessary??
Our past and present lab computer systems (Sunquest and Meditech) won't let you allocate a unit if the retype has not been done. We do the retypes as soon as we enter them into inventory.
As with the others above, we haven't used retype stickers for years.

Dump the labels. This is the 21st century.
