plasma: 2+reactions with all e+ cells in panel

eluate:Auto-Anti-e (acid-elution with BAG)

needs RBCs,

we should do an auto-absorption to get the auto-anti-e out of the plasma before we use it for xmatch to rule out an underlaying alloantibody, right?

without absorption autocontrol is 2+, xmatched rbcs 2+

can anybody tell me a good (and simple,fast...) way to do an auto-absorption?

seperate the plasma, wash the pt.cells with warm 0,9% NaCl- and .....?? we don´t have any other elution system than BAG, so we can´t use eluated cells to do the absorption.

The patient has not been transfused in the past 3 months, right? Autoadsorption is more effective if you can use an enzyme mixture to remove the antibodies from the patient's cells and make them better at adsorbing antibody before you try to do the adsorption. If the antibody is strong, you often need enough patient cells to do multiple serial adsorptions and that quantity of cells is hard to get from a patient that has hemolytic anemia.

A mixture of enzyme (papaine) and DTT (a chemical not a enzyme) works the best (according to Petz), it will ZAPP that antibodies from the cells.

I am a big fan of the PeG (polyethylene glycol) autoadsorption technique. It is fast, efficient, cheap, and doesn't require any special reagents other than a routine PeG additive (we get ours from Immucor). The procedure (from the AABB Tech Manual) is attached.

We started using PEG autoadsorption this year and it has pretty much replaced doing ZZAP-adsorptions.

It generally works great!
