    More often than not, but I like to operate by the Boy Scout motto - Always Be Prepared! The extra work is the cost of doing business. Sometimes it pays off big time, sometimes it doesn't.

    We still use the labels even though we've had an LIS which prevents release of un-typed units for two decades, our blood supplier already double types them, and we keep unprocessed units in an entirely different refrigerator. Anytime I discuss getting rid of them there's intense fear and paranoia. "What if there's a downtime? How will people know the units on the shelf are REALLY retyped?"
    I'm hoping to hear from more incredulous people who didn't even know others still used them to inspire me to forge ahead and suffer the backlash.

    Nobody on this thread, or any other thread is doing a computer cross-match.  They are either doing a serological cross-match (which can include an "immediate-spin" cross-match, albeit, it is not an "immediate" spin") or they are doing electronic issue.  A computer does NOT perform a cross-match!

    My facility is regulated by New York State Department of Health, and reinfusion procedures must be overviewed by our Medical Director, and falls back under my supervision.

    The 2016 Last Chance Review webinar and handouts is available for $80 each.  Granted, not free, but a good investment.  I attended the seminar before I took the exam and it was a really great review.

    Availble on AABB website; this is a GREAT study book.  Sample questions; not only do they have the correct answer, but they explain why each other answer is wrong.  I learned a lot with this book.

    They're still applying an incorrect interpretation of CLIA as it pertains to Transfusion Services (immunohematology).
    All blood bank activities are high complexity. High complexity testing doesn't have a 'technical consultant' designation. They have technical supervisors and (without re-reading) only MD, DO, and maybe doctor of podiatry(?) qualify. Technologists who Lead/Supervise/Manage/Senior Tech (however your organization calls it doesn't change the CLIA interpretation of personnel) have to be qualified as general supervisors. General supervisors may perform competency assessments. If you qualify as testing personnel and have two years of experience in high complexity testing (which it sounds like you do), you are qualified to be a general supervisor.

    No reg or requirment that I know.   It just has to happen before transfusion.   Some hospital policies state before it is picked up to curb wastage.

    If you are certified as a MLS, and have a BS or a BA, tell the powers that be you already have a bachelor's in one of the "four major sciences".
    Note that you are actually working as a general supervisor for BB (as pointed out in a post above).  You don't need to be a "technical supervisor".  That would be your pathologist.

    We do 7 days presurgical patients. In-patients: 3 days, day of collection is day 0 (i.e. 96 hours).

    I see from where you are coming Scott, but what REALLY annoys me is people like this nurse who assume, with little or no evidence, that other people are not as well-trained in some areas (such as aseptic techniques), as they are themselves, and then make statements to this effect, as if it makes them feel superior.  In other words, people who think they know better than others how to do the other person's job, and assume that the other person has no idea how to do their own job, just because the nurse (in this case) did not see what had already gone on in the background.
    As somebody said to me at a symposium this week, patients and doctors involved in a case with complex serology, only see a swan swimming serenely on top of the water when we produce compatible blood for transfusion, and don't see the feet working frantically under the water.  In the same way this nurse assumes she sees everything, but actually knows nothing.
    I was only half joking about irradiating her!!!!!!!!
