Working in a children's hospital we only give ABO compatible platelets or type AB but our first choice is identical. In most cases we use ABO identical and we have a very detailed Selection of Appropriate Component policy. For patients who received or will be receiving BM/HPC transplants we give compatible based on matching donor and recipient types, or type AB only (example patient type A receiving type O transplant gets either A or AB platelets). Also our listed ABO incompatible heart transplant candidates we will only give type O RBC and type AB plasma containing products until transplant and then ABO compatibility is determined by donor to recipient type. I attached our transfusion guidelines for non-transplant patients and BM/HPC transplants.
It has been over 15 years since I've worked in an adult hospital so I'm not sure how hard it would be to implement our policies but I know we have a white board where we keep track of the platelets so we can easily look and make sure we have what we need at a glance.

We require another sample of a different collection event, prior to transfusion.
Believe this is in the new AABB standards.
The computer automatically orders a no charge forward and reverse retype, if the pt has "No Hx".
We actually found a mislabeled BB specimen, once. Discovered when tech did the rpt on a diff sample and got a diff aborh.....

2.2.1 Pathogen Reduced Platelets and Cold Storage Platelets are not to be accepted and must be returned to the supplier.
Why do you not accept Pathogen reduced plts?
We are switching over to Psoralen treated plts, but have never transfused plt to any baby.

Yes, ARC is changing the ISBT paradigm and putting antigen typing results on the bag label (except after hours apparently when it will be on an accompanying paper only).
Rumor has it that Puget Sound Blood Center in Seattle is now molecular typing their donors and making the historic database available to their customers. That means you can check the supplier's data base, choose two historically c neg units out of your own inventory to type and not have to screen 20 to find two nor order them from the supplier. How sweet is that?!

Yes, we use a separate blood bank ID bracelet. We also use a waiver form for pre-testing up to a week before surgery. Part of that form is patient consent to wear bracelet until discharged after their surgery and a statement that if they remove bracelet they will have to be redrawn and will be charged for second workup.
