Hello everyone.  I am looking for a procedure for preparing reconstituted whole blood for neonatal exchange transfusion.   Does anyone have one that you can share with me?

We do this too, but the sops already posted have everything in them that is needed.   I would caution you to not use saline as a diluent.  We had a physician insist on it ages ago, but it doesn't

I would caution you to not use saline as a diluent.  We had a physician insist on it ages ago, but it doesn't work.  You really need the oncotic pressure from plasma or albumin.  If the concern is kernicterus, some places will use albumin.  That's not usually why we do an exchange.  We are usually confronted with a sickle cell patient with chest syndrome.

How is your final product labeled?  I didn't notice that in the document.

@Mabel Adams:  When we pool the units in WellSky Transfusion it creates a pooled unit label.  I recently added the list of product codes base on the starting RBC product code for staff to choose from.  My updated procedure is attached see 6.5.2.
