  1. We recently implemented TAR in Meditech 5.6. The requirement to have two clinical staff to check the unit of blood is now replaced with the barcoding process. Is there a regulatory requirement that the clinical staff verbally/check the compatibility tag to the donor unit.
  2. I would like to know how to manage the crossmatch for clinically insignificant antibodies. (lea,P1, M, and when you have rhig D My computer program automatically defaults to a AHG XM. Can I perform ISXM for these antibodies if the antibody is no longer demonstrating? Thanks, ESizensky

  5. Do you know what is JCAHO requirement for adverse reaction documentation. Is it required to document yes or no for reaction outcome for transfusions? I had an FDA inspector indicate that all transfusions required documentation for transfusion outcome . We currently updated to TAR and only adverse reactions are document .Is this compliant?
  6. How do you define the EMR ID for an irradiated product? Can you define an irradiated RBC as a Red blood Cell in EMR to match the order product RBC?

  9. HI Becky, I am having problems with apheresis FFP entering it into Meditech. We recently implemented ISBT. How did you create the different products for the same ISBT code. I have Meditech 5.66 ,.I can scan the product into Meditech E0869 but if the product is E0869AO, BO my digitrax label does not print the correct divided unit . I tried building a new product but I am prompted that E0869 code is in use with the main unit. I am would be interested how you have these apheresis products in your computer. Thanks, Eva

  13. We use A,B for confirming O donor units. Is it really necessary to even carry A,B anit-sera. My MI for my reagent states that a pos and neg ctrl is required for QC. Previously I used immucor reagent that did not require both pos/neg ctrl. Since we do not have any define use in our SOP would you discontinue the use of A,B anti-sera.
  14. Does any one know how to generate an ISBT pooled label for cryo? My current process pooling random cryo generates a pooled number not compatibile with an ISBT label format . Hemtrax printer does not recognize the pool number and requires ISBT number format. Eva

  16. We recently implemented ISBT. I would like to know how Meditech Users are performing "label verification". I am using the Digitrax printer to print labels. I only have a visual inspection for the labels. I am sure this is not in compliance for CAP. Thanks,

  20. I am validating the hematrax printer. Will I have to take in to account the different anticoagulants for each product modified? Could I just create labels for the modified product, i.e thawed plasma, irradiated blood. thanks, es

  22. Currently all my RBC products :04710,04730,04750.. are listed as RBC with no define anticoagulant. Modifying a red cell with AS-3 leukoreduced to an irradiated product does not print with the same anticoagulant. Do I need to create a new product for each anticoagulant ? I am trying to reprint the modified product label on a hematrax printer and the it does not mirror the original unit. Any suggestions?
