Forms for Issuing uncrossmatched blood

Hi, 

We are a trauma center and everytime we get an alpha trauma code, we issue uncrossmatch blood in a cooler, we give them 4 PRBC and 4 FFP using a form manually. We handwrite every unit and if they did not use the blood at all, they return the units and we have to rewrite them again. It is too much of work in rewriting, we can get 4 alphas in a shift and imagine hand writing it. It is a 2 page carbon copy form and the original has the back form for nurses to write the vitals. What do you do in your lab?

We are a level 2 trauma center -- when we get a full trauma paged, we pack up 2 uncrossmatched packed cells and have the unit information written on a form that goes with the blood cooler to the ED -- pretty much what you are doing, but only with 2 units.

We issue uncrossmatched units in our BBIS, so it prints tags for the units that resemble those for crossmatched units. Fast, easy and accurate plus the RNs can scan the units in Epic to document transfusion.  We use SafeTraceTx.  We have a downtime uncrossmatched blood form we can complete on paper.  We keep a photocopy.  See attached.
