  1. Malcolm is perfectly right, not that he needs my acknowledgement. To add: Cellano is the name of little k. The majority of people acroos all ethnicities have the k (cellano) antigen and not the big K antigen. Indeed you are as safe as can be, given the lab results that you posted, and the situation regarding danger to the baby is as if you did not have the anti-K.

  3. I agree to recognize and acknowledge the fact that you were thinking about the patient!! Great!! The sup could have explained it to you gently, all the while acknowledging your care. Well, we here do appreciate your patient-care !! You should be an example for your colleagues.
  4. Expiry date and time comes before the allowed 24 hours of storage for FFP in ths case so: at midnight the same day it is no longer considered FFP (after thawing). If you keep it beyond expiry it is as per AABB standards applicable to liquid plasma: "Liquid Plasma. Store at 1-6 C. Expiration: 5 days after expiration of Whole Blood. 21 CFR 610.53©applies"
  5. During my training I had to wait for my state license to round on patients with the doctors. I inquired why and was told that I may say something that might influence the doctor’s judgment and medico-legally, with no license, it could cause a law suit. So the sup is following the law and I do not think it is personal in this case. Good luck.

    Ok since tomorrow and all next week will be very busy catching up at work here goes. Malcolm has contributed a chapter to a book that was just published 2013 yes its out as 2013! Since I expressed interest as he was writing it, he has asked me to read it to see if I would detect a deliberate mistake. UNFORTUNATELY Malcolm I deleted all my Private messages to make more space too. So please kindly restate the name of the book and your chapter, for all to benefit. I said ""I will"" to reading it... no, nothing romantic.. sorry don''t get your hopes up, but it was a nice mystery though.

    Oh by the way Malcolm did you know that i never did attend the gathering and thus I cannot tease you about it. Clumbert this is not the secret. I ended up at Tufts ER due to severe allergic reaction, |I was taken by ambulance, they called 911 !!!!! Just like in the movies!!

  17. Any labelling error reflects possible patient misidentification and a new sample is requested. We do not, others may, differentiate betwen errors so there is no minor error. Any negligence in one part can reflect a major patient mis identification. It is very strict because Patient misidentification can obviously be fatal. We are strict and do not make exceptions, we do not work on a sample that is not 100%.

  19. We use triple name too, i believe it is a joint commission or CAP or both requirement.
  20. The segments were filled at collection so they should be whole blood. Reverse is not needed if done after ABO labeling, it is for confirmation that you perform the forward.
  21. Not sure this is where to ask, but here goes. I am looking for the names of various brands of 46.7 % Tri-sodium citrate that is in liquid form and sterile to mix with Hydroxy ethyl starch for granulocyte collection. Will be mixing 30 mL 46.7 % TSC in 500 mL HES. Thank you
  22. 1. Many times when there are minor errors in part of the name, the blood bank staff ask the nurse who brings the specimen to do the necessary correction and sign for that. Is this acceptable. No this is not acceptable. 2. The staff should retain the specimen and request and fill up the rejection form and send it back to the ward and to ask for a new sample and request. Is this correct? Yes or you can call them and document that in your log. 3. How do you classify specimen identification errors and what action need be taken for each type of error? You should have an "Incident Report" form with Preanalytical Analytical and Postanalytical And under each heading itemize what could happen. Then the tech just ticks that and writes the action taken. This also gets signed by the supervisor.
