    With all due respect, that means you have either "pulled the wool" over the eyes of an inspector who knows nothing about a blood group serology, or you have been lucky enough to come across an inspector who actually does know something about blood group serology, and realises that this kind of quality assurance of ANY panel (not just the Ortho panel) is nothing more than "smoke and mirrors" - and I am not blaming you for either scenario.

    I can't remember when we started immediate spin crossmatches. Started electronic crossmatches (sorry, Malcolm) in 1997.

    I had a tech tell me one time (long time ago) that he could not find K neg units. All of the units of RBCs he tested were K antigen positive. He was reading his tube typings microscopically! Probably a trace of another antibody in the antiserum (human).

    Definitely have to take note of manufacturer's instructions, especially important for antisera and special tests. Using a scope when testing with antisera is a strong compulsion in students and generalists that has to be weeded out. Not sure why that worries them more than the antibody screen or crossmatch, but it does.

    We've been converting FFP to TP directly for some time now.  All of our thawed units start with a 5-day outdate.

    Yes.  In the same way that use FITC-labelled monoclonal anti-D to estimate an FMH when the mum is D Negative and the baby is D Positive using flow cytometry, you can use FITC-labelled anti-HbF to estimate an FMH by flow cytometry when the mother and baby are either both D Positive or both D Negative, BUT, and it is a big but, there has, as far as I know, never been sufficient evidence that this is in any way accurate, but it will give you some idea (not that many Reference Laboratories in the UK will do this since both MHRA and UKAS have reared their ugly heads).  However, I have a question for you!  If you already know the baby's Hb (70gL-1, and not 70g/l incidentally, but in any case it should have been 70 g/L - remember, I will be marking your HSD papers!), why would you want to confirm the volume of the FMH?  It is irrelevant, in that it will not any difference whatsoever to the way the case is handled - the baby still needs a transfusion!
    IN terms of transfusing the baby, no, you just give the normal blood that you would give any baby.

    They have probably read something about the idea that a low titer - 4 or less - that doesn't rise is RhoGAM, not a 'real' antibody. We had requests for titers  for 3 or 4 months from a newbie OB/GYN doc who wanted to follow any of his patients who had anti-D reported post-delivery or post-miscarriage.   (We always report 'patient received RhoGAM on DD/MM/YYYY' on these patients to give docs a hint as to why the patient might have an anti-D identified, but his comfort zone apparently didn't go there.) He stopped doing it abruptly - maybe he was realizing that a lot of his patients had detectable RhoGAM, far more than could actually be sensitized to the D antigen or maybe one of his experienced partners educated him. 
    It might be worthwhile to have a pathologist feel them out about why they are doing it and to casually mention how much a patient is charged for a titer, suggesting a follow up antibody screen at 3-4 months if they really feel that they need to do something. If the patient is not pregnant, titers are not going to affect patient treatment/care.

    Does the hospital have a quality department or does nursing have a quality department or committee? Is your hospital accredited by The Joint Commission? If so, I would go to the quality departments. We changed our transfusion tag because nursing performed a quality audit and found that the information wasn't being recorded properly. They weren't recording the vital signs in the chart and they wanted them back on the transfusion tag. Now everything is in Epic.

    You can build a new billing code for the reference lab charge and add it in BOP to the TYSC. RLABID or something like it. Be sure to build the corresponding credit code in case you add it in error and you need to credit.

    We would sort out the specificity of the maternal HLA antibodies, and then give antigen negative platelets, with no cross-matching, as the babies are not going to be producing any HLA antibodies, and, if the mother does, it is not going to affect this set of twins.
