BPAM users: If you do not have papercopy of the transfusion record what goes in chart? How much information you have in the EMR for that unit. Is the unit blood type documented in EPIC? Is expiration date documented in EPIC?

if you are making whole blood in other words combining RBC and FFP in your blood bank it is consider manufacturing and you need FDA registration. Same goes for volume reduction of SDP? and removing adsol from packed RBC---might want to check on that one also.

we went with SafeTrace because of "lock down" feature. From the QA view, I like to have one way to do things instead of multiple ways. for same reason we went with Safetrace....

cthherbal---they are discussing downtime .....how can you do BLV during downtime?

sometime it may be the product is not used as intended....at least for us that was the case for freezer......our biomed technician has been with our hospital for >25 hrs and he used to cover the area of the fan in the back so he did not have to clean every quarter.
After so many discussion , finally bio med department head intervene and stopped practice....... because of cover/filter, the compressor was working too much and it died and had to replace it in less than five years...not anticipating any problems. we have two freezers, three double door refrigerators, platelet incubator, small ref. for ER, two plasma thawer and a cell washer....all helmer and very happy.....

Anyone had pooled cryo built in sunquest? Will you share the codes for Pooled cryo and thawed pooled cryo?
Do you use THC to thaw?

We require a redraw. For patients with trauma names, first, we ask that they not make the name change while we are transfusing furiously. We require the redraw as soon as possible after that but keep giving the units we had until we get the new specimen. The reason we redraw them is because otherwise, 2 days later, you have them hanging blood on a patient whose BB armband doesn't have the same ID on it as the unit. This gives them the idea that it is okay if the names don't always match when they are hanging blood which I think sets us up for errors in some other situation where, because of an error, the names don't match.
Almost same as Mabel: the only difference, our admitting calls us first and if the patient is critical and getting transfusion we do not allow them to change the name. we require new specimen after name change and we do not issue blood with new name crossmatched to specimen with old name...

DI is required with most BB LIS system. (interface with ProVue and/or Vision)

Wish you the best, Good Luck John.

anyone using safetrace --divide in syringe.
for me it is not working---with close system it keep same expiration for syringe. if I use open system it is changing parent code to open system code though the expiration stays same for parent and 24 hrs for syringe.

EPIC does not have 510K approved blood bank module so test orders and results can be processed from EPIC CARE to BEAKER to BB system and back to same route.
Product orders has to come directly from EPIC to BB system and crossmatch results from BB system to EPIC. (Product order can not come through beaker).
I totally agree that you need to be involved in EPIC beaker build...most IT department do not care for blood bank and they will do whatever they want without involving Blood Bank. They like control!!! total control!!!
