In the UK we are required to report all transfusion adverse events/ reactions to the MHRA via the SABRE portal. What I would like to know is if a patient reacts to some blood, this is investigated and no evidence of antibody involvement or indication of unit contamination etc found.Would you report EVERY suspected transfusion reaction to SABRE, regardless of the outcome of the investigation? I suppose ultimately you would have to ask would the patient have reacted in a similar way had they not been transfused with a component at that moment in time. Thoughts on how others are handling TR reporting to SABRE would be very helpful. Thanks!

Please can you tell me how haemovigilance reporting works in your organisation : What is the process and who is involved in deciding if an event is an SAE- and reportable to MHRA ? Thanks

  5. I read, an article about using blood not for transfusion, but for topical therapy, like in a case in opthalmology department, and a certain type of burn. what if there is a reaction in that kind of cases, does it part of the haemovigilance that should be reported, or not? anyone has ever reported something like that? nova jakarta, indonesia

I saw a wonderful case today of how, just when you thought you'd thought of everything, you see something new. I had an appointment today in a lab, but arrived early, so I went to get a cup of coffee in the cafeteria, which has a lovely terrasse - which I didn't use as it was too hot. the temperature today is 30°C. However, one of the patients was out there enjoying the sun - with his arm connected to a blood transfusion - and yes, I do mean red cells:cries:

  7. We all at some time, receive complaints from medical/surgical staff about delays in the provision of blood components, especially during a massive blood loss event. Generally, on further investigation these reveal communication problems between the teams involved. In the last two years (2008-2010), if you can recall any complaints that your department investigated regarding similar problems, were these reported to SHOT/SABRE? Thanks for your help!

I would really like to know how folk investigate transfusion reactions, particularly in the UK. Would a different screening technique also be included in the investigation or are folks just re-screening pre/ post samples and cross matching using the original technique? If the original screen was automated, is re-testing performed manually or re-run on the same analyser? Thanks!

Am I the only person in the UK who would really like MHRA and SHOT to streamline their haemovigilance reporting systems? Why can't there be ONE system in place- this is very un-lean and becoming even more time-consuming. Additionally we have to remember another set of passwords!!!!!

Many thanks again Cliff for arranging these additional forums. We are hoping to get folk discussing UK Haemovigilance issues here and would also welcome input on how the US Biovigilance program is coming along too.
