Displaying Plasma Expiration

How do different facilities display the new expiration of plasma after it has been thawed? Do you put a sticker with the new expiration somewhere on the bag, just on the transfusion tag,...? Any input is greatly appreciated!

My current hospital has the staff cross out the expiration date and then it is initialed by the tech. There is another label that is placed over the temperature storage area that lists the new expiration date and time and the appropriate temperature for storage. The testing time and date are listed on the transfusion bag tag but not the expiration date and time.

At my last hospital they have a revised outdate that covers the original outdate, with a new date and time. They have a secondary label that covers the storage temperature that tells everyone the new storage temperature requirements. The transfusion tag contains the date and time of expiration as well.

We strike the original (frozen) exp date and add a "Revised Exp Date" label above the bag label. The paperwork only reflects the thawed date and time.

We pretty much do what Larry does, cross out the old exp date and write in the new one. We don't modify to 5 day plasma, so we don't need to alter the labeling per se.

We convert FFP to thawed FFP. The thawed FFP will have the new expiration date/time. The thawed FFP ISBT label is generated and applied to the Plasma bag.

We edit the information date in the computer to reflect 24 hours after thawing (we do not convert to thawed plasma). The tag that prints out and is placed on the unit then has the revised expiration date. We also place a sticker with the revised expiration date over the expiration date on the bag. This way the date on the unit matches the date on the paperwork.

Right now we had the new expiration date on a smaller label added to the bags. We are in the process of making on-demand labels for this use.

for our codabar units we crossout fresh frozen and write "thawed" and also change the expiration date manually. For our ISBT units, we print 4" X 2" label (new Product code & expiration) label and apply it on the original label.
