Documenting Transfusions

I am currently updating my forms and I realize that our form for recording Transfusions is extremely outdated. Is anyone willing to share their worksheet or form for documenting Transfusions. ( The form will be used by our Nursing staff for our RBC Exchange patients.)

Any help would be greatly appreciated.

Thank you for your document form. Now I have the urgent need of the form of blood product labels ( for products: FFP, Red cell concentration, pool platelet unit......) Any help will be greatly appreciated.
