Is anyone willing to share their worksheet or form for documenting Transfusions. adiescast Posted October 31, 2008 Share Posted October 31, 2008 We have the transfusion documentation on our "bagtag." It looks a little funny in proof format, but that is what I have. The bottom right is a sticker that we put on the back of the unit so that the bag is still labelled when the nurse pulls the tag off to document.
