I discovered half a month's qc papers were lost ---can't find our tube quality control documentation anywhere How do I handle this? anyone else have this happen?
We were very disappointed with this device. The +/- 2 degree makes it comparable to the infared Raytek thermometer. Not worth the expense and hassle of daily QC. We just boxed it up and put it away.
Per the AABB Technical manual--"Avoid using Bg+ red cells because they may result in falsely high values, especially with sera from multiparous women." Do you have difficulty finding cells that have the extended typing?
We are a level one traum center-all our "does" are assigned a medical record number -we use this as a patient identifier - If the Doe is assigned a name later--we still use the medical record number for previous history check oh oh--now we have a patient that we gave 9 units of blood --she has developed anti-Fya and anti-Jka and we find out she was previously identified with a different medical record number and had anti-Fya! We have resolved to look every patient up by both name and medical record number now Is that how you all would handle this problem?
10 years ago we removed our surgery refrigerator--per Jacho regulations. Now The powers that be at our small hospital want to put the refrigerator back in use. How many regulations can you help me with? I need to present a fact filled argument against this. please help
We are a small transfusion service. We have tested the ISBT products and have written the basic SOP's I am trying to write a validation plan. Does anyone have a site or template they would like to share? The last inspector mentioned fda.gov has a template? thanks for your help.
we use this for our competancy training and assign two techs an abid--and request they perform in two methods--gel and tube. grading reactions is not compared.
Extensive validation will be done on the products we will most likely receive. The Midwest ARC has provided a menu of items they expect to process. There are so many more products on the ISBT table. Do you just write into your SOP's--will be validated as they arrive?
Would you please help? We have not historically charged for the abbreviated panels we perform,--for passive D siturations or selected cell panels. Is there a CPT code we could use so we could capture these charges? They are a lot of tech work.
We have teh DH4---love it. routine maintence can be done by our lab assistants-easily. calibration instructions are a little challenge-- thaws the plasma or cryo very quickly, safely and---raises the product so show it is done so our techs can see it across the lab.
Transfusion therapy clinical principles and practices 2nd edition pages 173-175 offer advice. If the unit is more than 7 days old, the rbc's should be washed etc.
