  1. I'm interested in hearing if other labs have a delta check set up for INRs. If so, what is the delta and for what time interval.
  2. The Ortho fetalscreen kit states "The maternal blood should be collected as soon as possible after delivery." Our policy states 1-3 hours after delivery and the order defaults to "Stat".
  3. We have a provider who wants all of her pregnant women to have a repeat antibody screen sometime during the second trimester. I've never heard of anyone doing this on a routine basis. Has anyone else had this request? Thanks
  4. Thank you all very much for your replies. This has helped, greatly. I was leaning towards the "new type/admission" process, initially, and it looks like that's what many of you are doing. We have a patient population who have been known to swap identifications which is why I don't feel comfortable relying on historical typings.
  5. I am trying to determine the frequency that other transfusion services perform ABO testing on their recipients prior to FFP transfusion? Do you perform a new type if more than 3 days have elapsed? What about with a new admission? If the patient has at least two previous blood types, do you still do another one prior to transfusing that patient even though it's a new admission? Thanks in advance for your comments.
  6. The question is whether to continue to monitor the patient once the transfusion has ended or just let them go with a list of "reactions" to watch out for. We also follow the patients closely throughout the transfusion but we also like to check again, one hour post transfusion. Do you check vitals any time after the transfusion has ended?
  7. I realize this is an old thread but we are currently evaluating our post transfusion patient observation time at our hospital. We check vitals at one hour post transfusion for all of our patients. We are being asked, by nursing, to disregard this post check for out-patient transfusions at our infusion site. We would like to know what others are doing for all transfusion recipients and whether any distiction is made between in-patients and out-patients at your institutions.

  9. I know this is an old thread but we are now in the same boat as Jody. Has anyone ever used Cerner for validation of their own system? I'm not sure how I feel about that and am interested in hearing what others think. We will be doing extensive unit testing ourselves but we want a third party for anything that may get overlooked. Thanks in advance.
  10. Are the techs diluting the 3% Panocells when using gel? They may be using Liss in tube since 0.8% is needed for gel. Do you a 0.8% panel?

  12. Thanks for your replies. I guess we will stick to quarterly like it has in the Technical manual but I do think it's a bit much. That's just my opinion.
  13. I don't know if this has been discussed before but I'm interested in hearing everyone's comments. Many transfusion services verify the accuracy of their pipettes twice a year but the technical manual says to do it quarterly. What are the majority of you doing? Thanks

  15. Can anyone give me a resource to show that routine ABO testing of baby's from O moms is not necessary and a waste of money? I have to produce evidence as to why we are not doing it and why we don't need to be and I need it quickly. Any help is appreciated. Thanks :mad:
  16. We are just now starting to get into gel and trying to get used to the pipettors. I have both the electronic Biohit and the manual tipmaster. I'm not really thrilled with either one of them. Has anyone ever used any other pipettors for gel testing?
  17. How is everyone handling CAP standard TRM.31450 "Comparability of Instrument/Method"? Are you comparing tube, gel, and automation? Does anyone have a written procedure for doing this that they might like to share? I'm trying to decide the best way to do it and am looking for ideas.

    What do you do with your information once you have it? Do you follow-up on each one and do a root cause or do you set a threshold and then once passed do a more in-depth look?

    Is there anything special about the phosphate-buffered (approximately 15mM)? I use buffered saline at the pH they recommend but it says nothing on it about phosphate or mM.
  21. We are wondering how to interpret the standard that says a computer system must be validated on site. Does that mean that a third party validation is not good enough or does that have to do with a hospital system having multiple hospital sites? We are a bit confused. Thanks
  22. We only collect Autologous blood at our facility. When we process the units, we record the segment number. Is that a process that others follow? Do you do it electronically or manually? We are switching LISs and want to continue this practice. Thanks
  23. For those of you who do use BB armbands, do you have a specific color that you have to use? Is the color mandated by your hospital?
