I'm with you Terri.  We are required to do what is ordered not what should be ordered.  Maybe the physician knows something we don't and they don't want irradiated for this patient at this time.  Alot would depend on how the policy is written.  I suppose if tburl has a policy that makes the staff responsible for double checking everything the physicians order then it may fall under the reportable column.  I personally would get rid of any such policy if at all possible.
