keeping unit traceability record

respected fellows,

I need urgent help and feedback about how everyone is keeping unit traceability record in their facility and what are the essential criteria to be included and what is not allowed by accreditation bodies.In our facility all record keeping is in form of hard copy. I have designed the form and revised it too but during the past two visits, the inspector's expression were as if he has some reservations although he did not reject it completely.

You need to know everyone who had anything to do with the component; where it came from, what was done to it, and where it went. When I do inspections I ask the facility to select 5 components with final disposition and then I want to see that everyone, including Nursing and transport personnel, were trained to do what they did . . .

Our blood unit related records are all computerised, but we still keep hard copies of each unit record from donor details until final disposition. The same are bound into book and kept for 5 years as required by FDA.

thanks David and Aafrin for responding but actually I wanted to see the form or format in which the other facilities are maintaining their record. I can post to you people my own form so that you can correct me.
