Relabeling of Prepooled Cryoprecipitate and Plasma, Cryoreduced

Two questions:

1. Does Plasma, Cryoreduced also fall under the same category as FFP... does NOT require relabeling with a 24-hour expiration date?

2. What about Pre-pooled Cryoprecipitate? Since it gets a 4-hour expiration date, can the expiration date change be made on the original label or does it require relabeling? We are a Level I Trauma Center and I see the time needed to relabel being a problem by extending the turnaround time to the patient. We currently pool our own cryo, but are planning to transition to the prepooled product.

The expiration date is not required to be in machine readable format. Therefore, ANY product that only has a change to the expiration date can be re-labeled (legibly and with permanent, moisture-proof ink) in hand written format. All modifications require re-labeling, but not all re-labeling has to affect the entire label. The biggest problem I see is that if you are using ISBT128, then technically the component code changes when you modify the product in any fashion. The component code must be in machine readable format. This might be a good question to pose to the AABB Standards group (e-mail at standards@aabb.org ).

There is no requirement to re-label a thawed product if the only change to it is thawing, whether the labeling is Codabar or ISBT 128. For example, if you thaw and FFP and use it within 8 (or 24) hours, only the expiration date needs to change. If you extend the dating to 5 days, then it becomes PLASMA instead of FRESH FROZEN PLASMA. Then, you have changed the product (beyond thawing it) and you do have to change the product code label. (For facilities that want to use thawed codes in order to allow the computer to control expiration date/time, then using the thawed codes available with ISBT 128 is fine.)

For the two products in question:

PLASMA, CRYOPRECIPITATE REDUCED: If this is how the frozen product is labeled, and you just thaw it, you may just change the expiration date and time.

POOLED CRYOPRECIPITATED AHF: Again,if this is how the frozen product is labeled, and you just thaw it, you may just change the expiration date.

Thanks Pat - glad we're doing the right way!

Our thawed FFP is used as PLASMA - we relabel, but only use the 1/2 label.

Same as when we Irradiate.

We have a walk-thru list posted next to our Digitrax.

Starting ISBT code -> Ending ISBT code. Made life easier for the techs - and my life easier too, since we don't have 3000 products defined in our computer system!

There are codes for Thawed Pooled Cryoprecipitate. Depending on if it's prepared in an open system or not, the codes are::

E3591 Thawed POOLED CRYOPRECIPITATE|None/XX/rt

E3592 Thawed POOLED CRYOPRECIPITATE|None/XX/rt|Open

Codes for frozen pooled cryo are:

E3583 POOLED CRYOPRECIPITATE|None/XX/<-30C

E3584 POOLED CRYOPRECIPITATE|None/XX/<-30C|Open

If you get prepooled cryo and thaw it, you only have to change the expiration date/time on the label. This can be a hand written correction, as long as it's in indelible ink and legible.
