Group& screen and Blood request forms

Hi Everyone,

I need to re-design our request form for tests / blood component requests. If anyone has an example they could attach for ideas, this would be really appreciated.

Here's jpg image of our requisition. Though now that I look at it, it needs some revision - Hespan is no longer in the BB and some codes have changed. Still usable though. Just a 1/2 sheet. I've redacted pt info. Most of our orders are now EMR entries that download into the lab system.

Thanks very much for this Kate. It would be good if others could also attachtheirs too so we can can establish a good range of ideas, which I am sure will help many of us to further develop our forms.

All of our orders for tests and blood components come to us via the computer. The nursing floors, Emergency Care Dept, Surgery Dept, etc., enters their requests into their computers. The orders cross into the Laboratory computer system and then into the Blood Bank computer system. (So all we see is a little computer label.)

PDF of Request Form attached. Small change is we have added sex on patient details (oops) , but addressograph labels are usually placed on them anyway.

We get all of our orders electronically. When the computer is down, our downtime request form is a form that integrates several lab sections. Blood bank is on the same form as everyone else. I would post the form here, but it is VERY busy.

Thanks Eoin, nice form . You seem to have some very good documentation and systems in place...I can feel a need to go to Ireland for a visit!
