Patient Identification

At my facility we currently use the Securlink armband system when crossmatch samples are collected. The armband is completed at the bedside and all information is obtained from the hospital armband. I would like to find out what other facilities are doing. We have the occasional error in spelling or the ID number is wrong resulting in re-collection of the sample. Thanks!

We use regular armbands and we receive requisitions with the sample. Most of the time the req is stamped with the same stamp that is used for the armband, but sometimes they write the pt name/reg # on the req and it is misspelled...then we have to either ask for a redraw or check in a different computer system under "alias names." (i.e., sometimes even misspellings are under the alias name category).

We use an homemade wristband/card system similar to Typenex with barcodes. Only the date/time is written on the band. It is used in conjunction with hospital ID band. We do not have requisitions, Meditech labels are printed for collection.

Patient identification is a big issue. We have the policies in place, the education is provided and still...

So we are now looking into barcoding and bedside readers for blood administration. Does anyone have experience with this that they can share please?

We use the Hollister system (now Precision Dynamics) wristband system and will continue to use it until we go fully barcoded.

The wristband has a specific red 'R' number for each patient and the band for each patient contains the patient's full name, medical record number, and date of birth. So we have a nice safety net of independent identifiers (including the patient's hospital ID band, which is barcoded).
