Information on transfusion slip

What information do you require the transfusionist to document on the transfusion slip? We want to switch to a 4" x 2" label that can be affixed to a mount sheet. In addition to the Blood Bank information, the label will have room for the signatures of the transfusionist and person verifying the patient/unit ID as well as the start and end date/time. Vitals will be recorded on the mount sheet for med/surg floors, on the critical care flow sheet for specialty units, and on the anesthesia record for intraoperative transfusions. Nursing is requesting that the label have a place for the signature of the person ending the transfusion. This is on our current unit tags, but is it a requirement? It would be a tight squeeze getting it on the new label.

This should be required only if you consider this one of the steps in handling a unit for which we must be able to identify the person involved. I can see why this could be important to the actual record so if there is a problem, nursing knows whom to question, but couldn't it be on the flow sheet or mount record?

My theory was, if we are not going to get any slip back, then why do we need to supply it in the first place. Nursing was already using flow sheets on which they recorded most of the data all over again.

Our new labels will not have the space on them for any nurse signatures. They will merely identify the product and the intended recipient and state compatibility. They will be attached to the units. Our nurses will record everything on their equivalent of your mount/flow sheet. In fact, we won't even supply a spare label for our main hospital. They will stamp the flow sheet with patient ID and pull off a unit number sticker to attach to it.
