Transfusion Reactions-FFP/PLP

One of our Pathologists is questioning our policy on Transfusion Reaction investigations for non-severe reactions to FFP and Platelets. I have searched the manuals including the AABB manual on transfusion Reactions, but have not found anything that gives specifics on handling these cases. If anyone has any input or would like to share how they handle their investigations - I would greatly appreciate it.

All mild reaction such as hive, urticaria...we do not required a specimen. We just request RN to fill out the transfusion reaction work up form and we do clerical check in the blood bank.

Other reaction to FFP & PLT such as temp elevation, hypotension etc...we treat as reaction to RBC and perform full workup except crossmatch.

AABB standard 24th edition-----

7.4.1.1...signs and symptoms suggestive of mild allergic reactions (eg. urticaria)need not be reported to thblood bank or transfusion services...

We had a NJ State inspection in March of 2006 and they are requiring us to do a full workup on ALL transfusion reactions. This includes type and screens on pre and post samples, and x-matches on all PRBC"S that were transfused within 24 hours, and all clerical checks and DAT'S.

We also had to revise our Transfusion reaction form to include all their new specifications. We also had to include a space for who was notified in BLood Bank and the time of call. They also informed us that they want a faster turnaround time. The DAT'S are done immediatley, and the rest is done usually within 2 hours, unless the DAT is positive and then we give it STAT priority.
