We had a NJ State inspection in March of 2006 and they are requiring us to do a full workup on ALL transfusion reactions. This includes type and screens on pre and post samples, and x-matches on all PRBC"S that were transfused within 24 hours, and all clerical checks and DAT'S. We also had to revise our Transfusion reaction form to include all their new specifications. We also had to include a space for who was notified in BLood Bank and the time of call. They also informed us that they want a faster turnaround time. The DAT'S are done immediatley, and the rest is done usually within 2 hours, unless the DAT is positive and then we give it STAT priority.
