  1. Could it be that the preservatives they are using are just not effective? Have immucor replied further to your problem?
  2. In the UK Immucor have issued a field safety notice about deterioration of plates if they repeatedly pass through the equipment. I will try and attach this to this thread on Saturday- when I get to work. If only they would standardise what they tell us all!!!
  3. Earlier this year someone in the UK mentioned having growth in their ABO typing reagents for ECHO. I think this could have been due to using these reagents for months and months, regardless of the azide being present. If your phenotyping reagents are being left on board for long periods or repeatedly placed back in and out of fridge there must be an increased risk of this occurring.

If you can collect stats on your current processes and for the IS screens that only showed cold antibodies, in addition to any info on delays in blood provision caused by having to investigate these, you may be able to agree on changing procedures. Surely the current process is more difficult for the cross-trained staff to follow through, and needs to be simplified for them too.

For users of the Galileo (not ECHO), there is a risk that the Capture-R screening plate could be passed through the incubator for potentially up to 12 times if strips are not deselected and removed from the plate frame, with the liklihood of deteriorating the screening cells themselves. This is far worse than re-incubating the gel cards.

Does anyone give protected time on a regular basis to staff for this task, or do you expect them to manage within normal working hours?
  12. The National Patient Safety Agency (NPSA) in the UK states that hospitals with an 'open' incident reporting culture are the ones that improve. Everyone must be encouraged to report these events without fear of blame and punishment attached. As Kate mentioned a causal analysis must be performed to see where the problems really lie. In the case jcdayaz has stated, with nurses constantly drawing the wrong patients, thisshould be properly investigated to see why, even after training this still happens. It could be due to staff workload or even the basic arrangement of the room and trolley etc where the phlebotomy is performed.

    Risk Assessments

    I am currently in the process of re-writing/ reviewing our risk assessments of various processes, problems etc . I'm not brilliant at this stuff, and learning everyday how to write these documents and apply risk scores. Does anyone have hints and tips for this process? Thanks
  14. We perform the decon using 'perasafe' supplied by immucor and have not noticed any of the problems you have. You could ask Immucor if this is the same as the solution you use, if not give this a go.

  18. Disciplining staff for not reading SOPs does seem to be the way things are heading. It will only really need to be done once and everyone else will start taking their responsibilities seriously. We haven't had to do this yet (not because they are signing these......but rather that i'm trying to get my head around this unpleasant aspect). We are all very busy anyway, and staff say they are not given any time to read these documents at work. I personally feel we can each make 15mins a day (at work even!) to just grab an sop and read it. how do others feel about this? It's very possible that folk are just not good at time management and we all need to learn to improve this.
  19. At my previous lab we kept the charts, but that was only because the system was new. Once you have confidence in the monitoring sysytem what is the point in monitoring using charts as well ? The electronic system data must be held/ backed up and be secured. You could print off a daily list of temperature excursions that you review, and possibly show evidence of weekly review of your electronic data. We are meant to be leaning our processes by using technology- not complicating these and creating even more work!

  21. I just tried this the other day- using Outlook (work email), I think voting options are in the 'tools'. Used this for a change control request....and the staff actually noticed this button!! So thanks dherbert for the info!

I would have thought you could do away with paper copies as long as your calibrations were as Eoin stated.You would also need to ensure (as for all IT systems) that the electronic data was kept securely and retrievable in the event of system failure ( so a back up copy possibly?). One of the many reasons for getting these temp. monitoring systems is to make the process more manageable and to cut down on staff time changing chart records weekly. I also agree it would be a good idea to keep some charts to place back if the monitoring system fails.
