did anyone get a reply from service. We too are seeing the same. also, does anyone have a form they use to document service or error calls and follow-up. We just went live in November and realized it would be good to have a running log for this. any help will be appreciated.

Any help with these new questions regarding therapeutic phlebotomies would greatly be appreciated. We are hospital based transfusion service who perform TP's on inpatients and discard the unit. Our Medical Director oversees the program. It seems CAP is now wanting a plan, goals and indications. Do you think a physcian order set would help with this?
