PI Initiatives Form

I am currently looking for a form to track the progress made on PI Initiatives.

 

We have this horrible trend of starting these great PI Initiatives, but never following through.

It always seems to be either forgotten, no longer any issue, criteria have changed, etc.

 

I am hoping that someone has a form that I can present to the staff as a way of documenting their progress on these PI Initiatives.

 

Any ideas or recommendations highly appreciated.

One suggestion: put things in chart form when possible. Easy to see visually if something is improving or getting worse.

We have a PI bulletin board at work where we post things like this to let the staff know what we are monitoring, as well as to encourage the staff when things are getting better.

See attached for an example. We were not getting great compliance with our shift-to-shift communication in the Blood Bank, so we started monitoring it.
