Transfusion Monitors/Competency of Transfusion Service

How do you monitor the effectiveness of your transfusion services? An example would be following a unit to the floor and making sure all the requirments are documented - patient ID, 2 personnel verifying all the info, vital signs checked, no fluids running. Are there other ways to do this?

Yes, this is what we do. We audit one unit each month, we follow it from beginning to end and have a checklist to make sure all items in the SOP are followed.

A big issue I find during assessments is incomplete transfusion data . . . sample some completed transfusion forms for completeness.
