Transfusion Reactions and Clerical check

Similar to Denny - nursing draws the blood at our place so they draw the patient for the reaction workup, check recipient ID against the XM forms, document this and send it all down to us. We check the donor ID against the forms when we get the blood and paperwork returned.

Nursing staff do clerical check at their end (and sign the transfusion reaction form to say it has been done) and we do for anything we 'touch'. In both cases it is done by someone who wasn't involved in any of the process to reduce the risk of a 'cover up'.
