Rhogam screen

Long ago and far away (in another galaxy), there used to be a little vial of RhoGAM intended for crossmatching with your patient in the box with the syringes of RhoGAM. That was discontinued many years ago.

Our workup includes Rh type, antibody screen (which I would like to drop), fetal screen and Kleihauer-Betke if the fetal screen is positive.

I have an additional question relating to this topic. Our ED physicians want to be able to give RHIG based on previous records. A few years back, we caught a case of identity fraud because we did repeat the type while the patient was in the ED. I am hesitant to drop it because of this. The current CAP checklist says there has to be a type on file by a "CLIA licensed facility" and the AABB is vague, so I'm afraid that if this really gets pushed, I won't have a leg to stand on.... Thoughts????

We are currently wrestling with this, too, with ED sometimes calling the Blood Bank tech and asking them to look up the patient's previous blood type. This makes me shudder! Some of you wouldn't believe the incidence of similar names, similarly-spelled names, or identical names (some even with very similar or identical birthdates!) I, too, discovered a case of identify fraud based on ABO/Rh test results. (This happened many years ago, but I am told that identity fraud / insurance card fraud is on the increase as more people in the US lose their insurance coverage.)

I'm confused about why some of the ED docs are willing to take this liability risk in order to save the cost of the ABO/Rh test, especially in view of how much money I see being wasted by ordering so many unnecessary laboratory tests because they can't interpret the basic screening tests properly or all the elaborate tests they order to "cover their posterior."
