Monitoring AFTER transfusion

We were cited for not having a policy for post transfusion discharge instructions and how long patient's should be monitored after transfusion. Our Blood Administration form has a spot for 1 hr post transfusion vitals, but states for RBC's only. All other products only requiring initial and 15 minute vitals.

Couldn't find any references. Any suggestions.

1. How long to monitor after transfusion?

2. Do only patients who are discharged in less time than established length of time for monitoring qualify for discharge instructions, or do all patient's getting transfused get the discharge instructions (if they have a transfusion reaction at home)?

We also use the 1hr post vitals, but I was unable to find any reference for that when the policy was last revised. Computerized transfusion administration records have a hard time complying with a vague time, so it's up to the nurse to remember -- it's rarely done, on audits.

We have a post-transfusion information form for our non-inpatients, and it's documented with the discharge nursing instructions. Most of our outpatients are oncology patients.

I have seen several "Inpatients" discharged immediately after transfusion. There must be some kind of guideline for how long to monitor.

Our pathologist feels that the safest thing to do, is give ALL patients that have been transfused, discharge instructions.
