We put a list with everyone's name in the very front of each manual. Techs date as they read the manual. Any manual not dated is brought up at review time, and the tech written up if not completed.

As it is a deviation from your written policy/procedure, you could have asked them to sign for it. Whenever you ask an MD to sign for blood, we have discovered, they tend to think twice & usually go with what we tell them. That or we can sick our pathologist on them.

We have both CAP & AABB and will continue to do both. We have the CAP as the rest of the lab areas want it, we in the blood bank would prefer to drop it. One of the main reasons is that we're afraid that if we drop a certification, our local competition will be more than willing to point it out to people as a way of gaining their business. Sad but true.
