Lab Incident/ Deviation system

From some of the comments made on this forum, there seems to be a problem with staff being able to formally 'complain', report, or highlight problems to the senior transfusion team . I would like to find out how you would report these events within your laboratory.

The deviation system we use allows all incidents, events, staff concerns, customer complaints, non-conformances on audits etc...anything, to be captured in a single system, so that it is formally recorded and may require corrective actions.....which may just be talking to staff about their concerns.

How do you report these issues in your labs?
