Donor implicated in TAD/TTD

We have a policy when a donor is implicated in a transfusion associated disease/transfusion transmitted disease (TAD/TTD) AND they are one of 2-10 donors, we place a surveillance deferral code on them, meaning that they are free to donate without restrictions. If this happens to them a second time, we consider it to be more than coincidence and we permanently defer them. Naturally if they are the sole donor we permanently defer them the first time.

My question is, does anyone recall a requirement to have such a policy?

The AABB Technical Manual (p 703, 15th ed) uses words such as "should be appropriately investigated" and "possibly defer permanently", I was just curious if their is an old FDA requirement related to this. I could not find one.
