  1. We do not accept the spiked units or left at R.T. > 30 minutes. At our facility,to control the wastage, the RN must file an on line incident report with the reason for wastage and return the unit for proper disposal. We discard the unit in blood bank and update the computer.

  3. To all Blood Bankers: This question is related to the use of diluent to make 2% suspension of RBC for Antibody Titeration. Is any one using 22% Albumin to make 2% RBC suspension for Antibody Titration? If so, How is the suspension made? The 16th edition AABB Manual does not specify to use albumin. In the14th edition AABB Manual, albumin was allowed to make a suspension. When technical manual does not specify the albumin, I presume only saline can be used. (My old policy,written by former supervisor, has suspension made by albumin) I validated the SOP by using Albumin and normal saline to make the 2 % suspension and albumin gave one titer lower than the saline.   4. We just had a CAP inspection. We do parellel testing with Manual Gel Vs Tube Method for Type and Screen and Antibody ID. Our primary method is Manual Gel and secondary method is Tube. We run several patients and compare ABO RH, Antibody Screen, and Antibody Panel by Gel and Tube. We include two or three positive antibody screen with different antibodies e.g. Anti D, E or K. The CAP accepted the parellel testing.
