It's challenging for us to look them up too. We are a large teaching hospital. We have at least 4 file cabinets full. We recently hired a college intern for the summer, part of her job was to look through all of the folders (about 11,000 of them) to see who was dead in our records, and who was over 100. We sent about 2,000 folders offsite. We added the birthdate to the folders so it will be easier in the future to see who's over 100.

Just a quick question also. Are you inoculating the bottles and then sending them to the testing lab or are they performing the inoculation step. Reason that I'm asking is because BioMerieaux states that the inoculation has to occur on site (not several hours before culturing starts)
I believe they want it in the incubator in 10 minutes or so. We fill the hard plastic "pouch" on the Trima bag, send that to our testing vendor. It needs to be put into the bottle within 6 hours.

Not sure where you're located. We send to Rhode Island Blood center for all of our testing, including BacT.

We used to allow directed units for our NICU and would do A or B titers. We now longer allow any directed units and only give O Pos or O Neg to the NICU. No titers, no washing. We transfuse a fair amount.

We're using the hard plastic "pouch" attached to the collection bag. You need to inoculate the bottle within 6 hours using those, so we send to a local blood center (who also does our testing).

Do you then have a policy that references the References which describes where the references are referenced?
Just wondering, Scott
Yes, in our document management SOP.
