Frequency of Laboratory testing for Biochemistry tests.

Hi anyone,

Our lab is trying to implement new tests like Serum theophyline, HBA1C Serum Iron and so on.

But before starting to implement these tests we want to run a survey questionnaire to all the doctors in our institution to know how often they need these tests.

The main aim of this survey is to know the demand for these tests.

The question of concern is whether any user has such a kind of questionnaire form. SO that we could use it as a sample.

Any help on this would be kindly appreciated.

Can you not estimate the test volume based on how many times the tests have been ordered in the past?

My intention is to conduct a survey so as to know the frequency that the doctors want the specific tests to be ordered on their daily basis patients.
