Tracking bad sticks and the need for retraining

I would like to pose an interesting thought. We track technically unusable (TU) units by employee number. If an employee has a bad venipuncture and another team member comes and "saves the stick", there is no way to record the assist. The original team member gets the "credit" for a good stick. That then shows as a favorable "TU" rate at the end of the month. However, the team member might really be in need of retraining and the numbers don't reflect this. Any ideas as to how to track those employees that always need an assist?

How is that data gathered? If the TU info is gathered by manually looking at the donor forms, then could the successful employee co-initial the "Venipuncturist" area? If you gather the info electronically, could you incorporate an additional field where the successful "Venipuncturist" could document successfully repositioning the needle?

If neither of these work, what about just "spot checking" a few days a month by simply capturing the data manually for that complete day in the donor room (or mobile)? That would take a little more human time to capture and analyze, but it should bring to light those venipuncturists whom need more training.
