Does anyone know of any regulation or requirement for transfusion services to receive back a completed copy of transfusion slips post-transfusion that are attached to units? Thanks.

We use a Cell-Safe transport container to issue blood to the OR. We apply a HemoTemp II indicator to each unit that is irreversible above 10 degrees C. How does everyone address the requirement to record the temperature every 4 hours or does applying the indicators solve this?

We do a "mini" panel to rule out other antibodies when we detect antepartum RhIg on antibody screens. If it is negative and we have confirmed the injection of RhIg, we report "Anti D detected. RhIg given on XXX date. We make no suggestion as to whether it is "due to the injection". We let the clinician make that decision.

We currently only have MT's working in the transfusion service. However I feel that there are some MLT's that are more qualified than the MT's that I now have working for me!

We use a commercially prepared Rh control on all AB positive patients.
