  1. We are currently being inspected by the FDA and the Inspector wants us to do a retrospective validation of our irradiator to test how many unit can be irradiated in one cycle. The manual says our GammaCell 3000 Elan will irradiate up to 3000 mls (thus the name) in one cycle and we have stated that we will only place as many units that will safely fit into the cansister and do not state a maximum number of units. We attach indicators on each unit. Has anyone ever performed such a validation? I'm rolling my eyes a little on this one
  2. We decided it was best to use the secureity code on our Trans-Logic system because we didn't want the blood sitting for any length of time once sent to the nursing unit. With the sercurity code the tube system Beeps until you put in the code, then the tube drops, so the blood won't be forgotten about. We use our BB phone extension for the security code
  3. For ISBT I created (inconjunction with our blood supplier) a local "D" ISBT codes for the packed RBC and FFP products used in the exchange unit. We modify the original products to the unique "D" product codes then use our computer pooling function to pool the products. The final Reconstituted Pooled Whole Blood product also has its own Local ISBT code. Our blood supplier reconstitutes the units for us as we have no way to hyperpack a unit of RBCs ourselves; They supply us with the original unit numbers and product codes of the units used in the pooled unit, then we "pool" the unit in our computer adding our own Facility ISBT DIN to the Exchange unit.
  4. I wouldn't get too concerned with IgM anti-M. The aforementioned article does not state the Ig classification, I would assume the Anti-M causing the HDN was IgG. That's why it is important to determine if anti-M detected in OB patient's is IgG or IgM and the IgG portion titered.
  5. You could create the phrase in Hematrax under special testing on the Expiration Date window.
