I have not taken that exam (CQIA), but know from the ones that I have taken that ASQ typically posts a list of resources / books on the website under the BOK (Body of Knowledge) section for each certification.

As an assessor, I keep my individual membership current. I find that performing assessments are just as educational for me as they are the facility.

The short answer is that the date format you use must be defined within your facility.

Just my $0.02 - if you are using a computer program to "make decisions" about donor eligibility, it needs to be Part 11 compliant AND validated. From what I have learned from several IT folks is that off the shelf Microsoft programs are difficult, if not impossible, to validate.
