  1. What are your practices transfusing A2 patients. Do you automatically transfuse O type blood or do you wait until that patient presents with an Anti-A1?
  2. With the revision of the CAP Checklist they now have the ALL COMMON Checklist standards. As a blood bank how do you fulfill the Method Performance Specifications relating to sensitivty/specificity, reference ranges,...Do you refer to a general laboratory procedure that is more for chemistry and hematology testing or is it not applicable? Any feedback would be greatly appreciated

I have had some feedback recently from the nursing staff who generally work with our outpatient oncology patients that if they have a patient who has a rise in temperature during the transfusion that the physician says to keep transfusing as it is most likely due to a neutropenic fever. However; I am concerned that they may be missing something else without doing the Transfusion reaction work-up. Does anyone have a procedure that addresses this situation?
  4. Our patients armbands are kept with the patients chart and banded when they present for admission--patients are not sent home with armbands.
  5. 3 questions... 1. Is there something in your nursing procedures stating that a transfusion should be started slowly the first 15 minutes and what is the rate? 2. Do you routinely premedicate with Tylenol and Benedryl? Possibly masking transfusion reactions? 3. What is your policy in starting transfusions in patients that already have a fever? Any feedback would be appreciated
  6. How long before a patients surgery do you allow them to come in and be drawn. Our current practice is 7 days prior as long as they sign a statement that they have not been pregnant or transfused within the last three months. Our pre-surgery team wants them to be able to come in up to a month before. Thoughts?
  7. I just saw something regarding gel crossmatching. If gel crossmatching patients with antibodies you have to perform an ISXM for ABO Incompatability. We currently do electronic crossmatch which determines electronically ABO Incompatibility and there are hard stops in the computer for it. Is this good enough so as not to do the ISXM?
  8. Does anyone know if there is a recommendation for doing a post-partum hematocrit on Labor patients
  9. Instead of doing the 24 hour outdate for thawed FFP, does anyone go automatically to the 5 day outdate?
  10. I do not currently work at a donor center so I am not familiar with the determination of platelet yield. I do know a standard dose is 3x10^11 to 6x10^11. We currently receive platelets with a platelet count and platelet yield listed on the unit. Can someone explain the differenc between these two measurements Thanks
