What protocols do you use for Lupus-Type anti-phospholipid testing?

We have been sticking with the DRVVT from IL, and reporting a sensitive APTT along with it. The interpretation is based on the results of the DRVVT though (Its a screen and confirm test).

Now our pathologist has directed us to also send out any positives for a Hexoganol assay for confirmation. Interestingly, another hospital in our town now does what we have been doing-reporting out the DRVVT screen results and leaving it at that (they used to send out the Hex as a reflex).

We use IL product here - we report a normalized ratio based on screen and confirm results. I know the ISTH is now recommending negative DRVVT tests be confirmed with a more sensitive test - like the hexagonal phase.

We have not changed any of our practices.

Can you cite a reference for that? Our pathologist has us doing the opposite for confirmation. We only confirm the positives.

(Also, IL has an algorythm using the SCT with thier LA screen and confirm which does not call for any extra confirmation.)
