Plateletpheresis in dual bags

Recommended Posts

If a plateletpheresis is collected in one the the "2 bag" systems (which have to be pooled before transfusion) should the expiration date be changed to 24 hours? It seems that this is a closed system and the expiration date should not have to be changed.

A closed system is only one piece of the puzzle. You also need to consider the platelet content and plasma volume of each individual product. These criteria are set by the collection set and equipment manufactuers and are related to how their systems/processes were licensed to maintain product viability during the storage period (CO2 gas exchange to surface area, etc.). ..hence the reason some blood centers attach a label indicating the 24 hour expiration date once the contents are combined into one bag. And to further compound the issue, different manufactuers have different content and volume criteria.....

If you are asking if you have to change the expiration to 24 hours once the two bags are pooled (assuming they are kept separated during storage because the single bag capacity to offgas CO2 is exceeded), the answer is yes, at least when using the bags I use to collect platelets. My manufacturer has a 400 mL limit for one bag (as well as a 2.1 million platelet count, but that's another story). Once you combine the bags, you no longer have the capability to keep the pH within range.
