Hello, We are in the process of redesigning our patient result reports (i.e. result reports that will be placed in the patient's medical record). We are interested in placing a "Confidentially Statement" at the bottom of our report. Does anyone have an example of a statement that they are willing to share? Thanks, Kim

Cliff, We are building a new laboratory system for our multiple purpose laboratory. This is the first endeavor for our vendor in a laboratory site. Therefore, they are not providing any validation suppose (ie. test case studies, guidelines, etc.). It is left up to us to test the system and make sure it does everything that we need. So.... What I am in need of are any general forms/documents that could be used in multiple areas to organize/simplify testing data. Something like this may not exist. But.... I just had to ask. Thanks, Kim
