I understand that a lot of donor centers no longer produce non-additive RBCs and was wondering what product was being used for large volume neonatal RBC transfusions, including PUBs and whole blood exchanges.

I was sent an article about using AS3 for exchanges (attached), but our neonatologists want more data before they will agree to change.

Moderate-size pediatric hospital (about 450 beds):  We give adsol for all of our routine transfusions (10-20ml/kg).  We still use CPD/CP2D red cells for all large volume transfusions such as ECMO prime and open-heart surgery and make reconstituted whole blood (from CPD/CP2D red cells) for neonatal exchange transfusions.  It does sound like this is becoming more of a luxury, as many Transfusion Services are not FDA registered and making the RWB product.
