We want to conduct a survey of our hospital customers (Physician, RN, PA) and I am hoping that someone out there has a survey that would be willing to share. 

Haven't done a customer survey for the Physicians etc but have done one for the nurses etc in the

Cancer Treatment Center.  Had questions about TAT, phone interactions etc.  Pretty short survey but

was afraid if it was too long they would not take it.
