Have you seen a positive Rh control test using ORTHO's A/B/D Monoclonal and Reverse Grouping Gel Card? Were there any other unexpected test results associated with the positive Rh control, e.g, ABO discrepancy, positive antibody screen, incompatible crossmatch? If so, what was patient's diagnosis?

We saw this on a patient with a very stron cold agglutinin.  Amazingly, the antibody screen came up negative, so it must have been easily warmed away, but the reverse ABO was messed up.  Prewarming the cells and running the blood type in tube was successful.

We wash the red cells in warm saline and then do the Rh type (and ABO type) in tubes, including a control, and, usually, it goes away.

What does a ORTHO's A/B/D Monoclonal and reverse grouping gel card test for?

 

It is used to test red cells for the A, B and D antigens and plasma for anti-A and anti-B. These test results are used to determine adult patient's ABO and Rh blood groups.

We've seen it in patients with CAA. I think one of our hyperviscosity patients had a positive control as well.

Brenda - which Quotient anti-D are you using?  They have 2 IgM versions and a blend.  I think only one of them will detect DVI (and at immediate spin too).  Just curious as I am considering purchasing some of their product.
